Certification of Physical Fitness to Participate and Liability Responsibility *

1. I understand and acknowledge that my participating athlete/team may incur personal or bodily harm while participating in sports-related activities, including but not limited to activities described as running, jumping, physical contact, etc. By submitting this agreement, I assume all risks inherent in these activities and accept full responsibility for any and all damages or injuries of any kind. I further acknowledge that to minimise the risk of injury, my participating athlete agrees to obey all safety rules and to report fully any problems related to his/her physical condition to a 55 Buckets Coach or Trainer as soon as the problem arises.

I understand and Agree to the Terms Above

I certify the following:

a. My participating athlete/team is not currently under the care of a physician for an injury or illness that may prevent or would prevent his/her safe participation in The Gauntlet Tourney. b. My participating athlete/team is not currently being treated or recovering from an orthopaedic injury that would prevent his/her safe participation in The Gauntlet League, Tournament, or Skills Training. c. My participating athlete/team has no history of fainting or other problems related to strenuous exercise; d. My participating athlete/team is in good health and there is no reason he/she cannot safely participate in strenuous activity. I understand as the parent/guardian/coach of the participating athlete/team, I am responsible for my athlete’s medical coverage and insurance to cover any physical injury, or illness.

Emergency Medical Authorization In the event of an emergency where I am unable to be reached, I authorize 55Buckets, J-one Training, The Gauntlet staff or representative to obtain consent to medical treatment deemed necessary for the safety and well-being of my athlete/team. I accept full financial responsibility for any medical care or transportation required. Photo/Video Release I give permission for photos or videos taken during training sessions, tournaments, or events to be used in promotional materials, including social media, marketing campaigns, marketing campaigns, and digital/print media. I understand that my athlete/ team will not be compensated for such use Refund Cancellation Policy I acknowledge that payments made toward participation fees are non-refundable, regardless of voluntary withdrawal, injury, scheduling conflicts, or removal from the program due to violation of rules or policies. Code of Conduct Agreement I understand that athletes and parents/coaches are expected to conduct themselves in a respectful and sportsmanlike manner. Any behavior deemed unsafe, disruptive, or disrespectful may result in removal from activities with refund. Release of Liability I do hereby release 55Buckets, J-One Training, The Gauntlet, Forza, Box2U directors, staff, employees, agents and/or volunteers and agree to hold them harmless from all claims whatsoever arising from the conduct of any persons which results in the injury or loss to my participating athlete.
